Ok this is going to be quite long-winded as the fight itself is very complex. Also feel free to post any observations I may have missed.
PHASE 1 -The fight starts with Twintania in the front with 3 adds Behind her. Main tank initiates pull while Off-tank runs through her and groups the three adds up in between the First and Second Finger(counter-clockwise). These need to be burned down in order quickly.
Adds will cast LIQUID HELL around every 12 seconds at a random player forming a large pool. This needs to be moved out of quickly.
-The Main Tank should hold Twintania between the wrist and the first finger and slightly towards the center to allow for group positioning once the adds have died.
Twintania will cast DEATH SENTENCE approximately every 35 seconds for the remainder of the encounter. Healers and applicable mages NEED to alternate virus on Twintania before she finishes her cast. The damage will increase with every attack until it reaches max potency. It can be reset with death but is not recommended.
DEATH SENTENCE will apply INFIRMITY past phase one which will decrease the healing the target takes significantly. Stoneskin along with Adloquium must be cast at this time to negate large portions of Twintania's damage.
PLUMMET will hit for a large amount of damage in front (conal radius) of Twintania periodically throughout the fight. This will persist throughout the fight and as such; no target should be in front of her besides the main tank at all times.
NEURAL LINK will also be cast just prior to phase 2. Phase two begins at approximately 90%. Before the transition into phase two DPS needs to hold and Mage ballad should be used to top off healer mana and ensure the proper placement of Neural link.Neural link is a black circle on the ground with a blue circle inside of it. This may be used ONCE EACH PHASE to negate a single hit and will become extremely important during the last phase of the encounter.
-The positioning should consist of Two groups on each side of the tail with a healer in each group and the Off-tank in one as well. Dps should be careful not to push Twintania into phase two prematurely (largely dependent upon healer MP, they will need it to be topped)
PHASE 2 -Once the first Neural link is placed Twintania will now have two additional abilities. Each ability has a distinct graphic and allows time for proper placement if required. Twintania should be repositioned to in front of the second finger (counter-clockwise again) directly after the first fireball is cast
FIREBALL (displayed as a red triangle over the target's head) deals a large amount of damage in an area divided by the amount of players hit by it. Additionally, for each player hit by fireball Twintania will gain a stack of WAXEN FLESH. For each stack of waxen flesh Twintania's Conflagration will detonate more quickly.
CONFLAGRATION (displayed as a blue circle over the target's head) disables the target and anyone caught within a large AoE of the target. This player and any additional players will be killed upon detonation. Conflagration can be targeted and killed as if it were a regular npc.
-The positioning in Phase 2 is the same as in Phase 1, with two groups on each side. Melee must stand at max range (her hitbox is quite large) and ranged and Offtank should stack accordingly.
Conflagration is to be placed directly UNDER Twintania and the tank should also move inside of the conflagration to negate a large portion of damage (seemingly cancels Twintania's attacks) while retaining your threat. This will save MP for the healers and must absolutely be killed before detonation. This is mainly used to deal with Infirmity, however, it may also be timed to coincide with death sentence to negate a large portion of tank damage.
If a Fireball occurs while a Conflag is up it should be placed inside of the Conflag otherwise the target should remain in group to spread the damage out. Placing a fireball inside of Conflag in this manner will kill the conflag much quicker.
Twintania must not be pushed into Phase 3 while a Conflagration is present. This is at 50% and shortly after the second Neural Link is placed into position.
PHASE 3 - As soon as the second Neural Link has been placed Twintania will fly up into the air. At this time the entire raid should run in-between the first and second fingers nearly touching the wall (DO NOT touch the wall just get close. If you touch the wall, it will touch you back and you will not like it) and stacked tightly. Shortly after Twintania will begin Dive-bombing a random player.
DIVEBOMB(displayed as a green circle over targets head) should me moved away from as soon as the graphic appears. The ENTIRE group must move away from the wall at the same time until she completes the action, in which the group returns to the original position and repeats. Twintania will Dive-bomb three times before she summons three adds.
Two types on adds will spawn at this time. TWO HYGIEIA which have relatively low health and ONE ASCLEPIUS which has a lot of health. The main tank should pick up Asclepius while the off tank picks up the two Hygieia. The Hygieia Should be focused down until the next wave of dive-bombs occur. This is at approximately 45 seconds from when Twintania spawned them. The Asclepius should be tanked through the dive-bombs (another 3 with the same movement as the first 3)
Upon Completion of her second round of dive-bombs Twintania will summon another set of TWO HYGIEIA to the far east. These must be grouped up in the center with the remaining Asclepius and a mage should use a LB3 to finish the Hygieia off quickly so that the Asclepius can be then focused down. Once the Asclepius is dead the main tank along with the SCH must move into the left Neural Link while the rest of the group moves into the right Neural Link. This is in preparation for the beginning of phase 4.
It is of great importance to not that the adds will debuff anything within a radius when killed. This debuff will increase damage dealt as well as taken and stacks for each Hygeia killed up to 4. Some groups will wait to kill all 4 Hygeia together will LB3 in order to burn Asclepius down as fast as possible. This of course will matter less if your group has outstanding DPS.
PHASE 4 -Twintania will renter the arena by instantly hitting the ground setting off Aetheric Profusion which she will follow shortly after with a second Aetheric Profusion (this is why you must be in the Neural Link
AETHERIC PROFUSION is a room-wide AoE that hits everyone for roughly half their HP. Neural Link is used here to negate the first cast completely.
-After the second Aetheric Profusion the main tank should pull Twintania to the center of the room a little below the middle of the wrist while the rest of the group spreads out in a large circle around her at max range. Any melee DPS and the OT need to be on each side of her tail. During this phase she will cast two abilities(along side death sentence).
TWISTER is a quick cast spell that target random party members. If you are caught in the twister you will be killed instantly. EACH member (including main tank) must run in a small circle as soon as she starts to cast this in order for it to miss. The only caveat is that this does not work 100% of the time and there is currently little information regarding the Twister mechanic. This is based off of observation alone but looks to be a solid way of avoiding it. When missed a small twister will be left behind that will initiate the same effect if crossed over.
DREADGUARD Hit's 1 shot, needs to be stunned immediately and will be summoned up to 4 times during this phase(more if your damage sucks ofc). He will be summoned focused on a single target,moving to that target, and needs to be slowed. If he reaches the target it's GG. If Twintania is positioned correctly he will appear right behind her.
The two above abilities will be rotated as apparent (T,D,T,D,T) and then the cycle will repeat so that you have two twisters in a row before a dreadguard is again summoned.
-At 30%Twintania will set another Neural Link down and this will begin Phase 5
PHASE 5 -Tank positioning does not change during this phase and Twisters are no longer cast. However, Twintania will now cast two new abilities.
HATCH (displayed as a black ball with a green grid) will be targeted at one random player every 12 seconds and then travel to the target dealing a large amount of damage in an AoE. The target will have a purple spinning graphic around them and will need to utilize one of the 3 placed neural links to negate the damage. If anyone is between the target of hatch it will instead hit them instead. This is where correct placement of Neural Links come into play. There is also a way of negating the need for raid movement into neural links by having the off-tank stand int he neural link under twintania to absorb it as soon as it hits the ground. However, this may become problematic upon a cast of her second ability, rapid liquid hell, upon the off -tank and will likely mean that the raid will have some involvement in neural link negation at least at some point during this phase.
RAPID LIQUID HELL in which she will target a random party member and fire off 5 Pools of Liquid Hell in quick succession. These pools need to be stacked tightly together in a U or circular formation; allowing room for movement around in case a member is targeted more than once. She will cast these every 8 seconds.
If The main tank or off tank is targeted it is recommended to stack all 5 in once spot if cooldowns allow.
-This is repeated until Twintania is dead or you are.

I have a question about how you guys deal with the Fireball. Our group is simply not geared enough, but we are having anyone who gets marked for fireball to run to the scholar and split the damage with the scholar and Eos. Is this the correct way? how do you guys do it?
we're doing it in a similar fashion, although we havn't tried in over a week. We were having Eos placed on the OT and just having that person run to eos when they were marked. If a conflag is down they just run to the center of the conflag QUICKLY, i was failing hard at this but meh, lot's for healers to track during this.
